Payment
Patients should be prepared to pay a simple copay or coinsurance if billing through insurance. Coastal Complete Care also offers reasonable self-pay rates for most services, which we would be happy to provide per your request.
Necessary Materials
In addition to the New Patient Paperwork, please bring in a photo ID, your insurance or Medicare card (if billing through insurance), and any referrals from other physicians (for Physical Therapy patients, or patients with HMO plans). Any other information in your possession that is relevant to your treatment would also be helpful.
